Vendor Stylist Agreement

By submitting this form, you agree to the following terms regarding any clothing or items ("Items") borrowed from ALIVE New York City:

1. Return Date
   All borrowed items must be returned no later than the agreed upon return date provided in this form or otherwise communicated in writing.
2. Condition of Returned Items
   Items must be returned in the same condition they were received — clean, undamaged, and in their original form or as close to original as reasonably possible. Normal wear due to fitting or shooting is acceptable, but permanent damage (tears, stains, alterations, etc.) will be considered a breach of this agreement.
3. Failure to Return or Damaged Items
   If items are not returned by the return date, or if they are returned in a damaged or unusable condition, you authorize ALIVE New York City to charge the full retail value of each item to the credit card provided as a security deposit.
4. Security Deposit Authorization
   By submitting this form and entering your payment information, you authorize a temporary hold of $1 on your credit card. This hold confirms your authorization to be charged up to the full retail value of the borrowed items only in the event of non-return or significant damage.
5. Identification Required
   A valid driver’s license must be uploaded with this form to verify identity. This will be kept confidential and used only in the event of dispute or non-return.
6. Acknowledgment
   You confirm that you understand and accept full responsibility for the care, return, and condition of the items borrowed, and that you agree to all of the above terms.
